Hospital-in-the-Home (HITH) initiatives are reshaping how healthcare thinks about hospitalization. Instead of treating patients within four walls, technology enables care at home, freeing up beds and easing pressure on wards. A recent study screened 2,631 scientific papers and selected 20 that describe management models for HITH. From these, nine models emerged, grouped into three categories: governance, general, and virtual. Governance models define decision‑making and resource coordination; general models outline workflow and clinical responsibilities; virtual models use digital platforms to monitor patients in real time. Moreover, researchers identified 23 operational strategies, divided into five groups: referral support, external support, care‑model support, technical support, and clinical‑team support. These guidelines help health executives pick the right model for their setting, cut costs, and improve patient outcomes. In practice, a well‑managed HITH can deliver personalized care, lower hospital‑acquired infections, and make life easier for those with chronic illnesses, contributing to a healthier, smarter city.

Governance models for HITH include centralized structures, where an expert committee oversees all cases, and decentralized structures that delegate decisions to local teams. The choice depends on hospital size and patient complexity. General models rely on standardized care pathways, with clear protocols for admission, assessment, and discharge to home. Some hospitals adopt a hub‑and‑spoke approach, where the central hospital acts as a hub for clinical supervision while peripheral facilities manage daily care. Virtual models leverage telemedicine, wearable sensors, and AI platforms to collect real‑time vital data. These systems can automatically trigger alerts to staff if patient parameters exceed predefined thresholds, enabling prompt interventions. Regarding operational strategies, referral support ensures primary physicians can quickly direct eligible patients to HITH, reducing wait times. External support involves partnerships with medical device suppliers and home‑care services. Care‑model support entails continuous staff training on updated guidelines and safety checklists. Technical support covers maintenance of digital infrastructure, data security, and interoperability between IT systems. Clinical‑team support includes forming multidisciplinary groups—physicians, nurses, physiotherapists, pharmacists—that collaborate to personalize care plans. Implementing these models and strategies requires cost‑benefit analysis, defining performance metrics (e.g., reduced admissions, patient satisfaction), and continuous monitoring to adapt the program to emerging needs. In summary, an integrated management, backed by advanced technology and a collaborative network, is essential for making HITH sustainable and high‑quality.
